A desire presupposes the possibility of action, and action presupposes the goal worth accomplishing. That is how Ayn Rand said it.
What she means is before you do anything you first need to have a desire that propels you into action. That action will be set towards someone's goal, so make your actions work for your goals.
This means objectively view how your actions are going to benefit you. Not to say that you shouldn't help other people, but the best business transactions are the ones that benefit both ends of the deal.
Physical fitness is not like business, you are only dealing with yourself. So you have to look at what is motivating you to work toward those goals you have in mind, and are those goals actually worth acquiring.
Many people build their desires on the premise of feeble goals, and maybe for a while they exercise toward their purposes but if the goal is weak then it will usually be forgotten or lost. An example of a weak goal is exercising to beat out competition.
What happens when that competition is not in your life any more, or if they give up? Are you going to be able to continue on your course without the other person's presence?
Find the motivation within yourself, instead of exercising for competition against other people do it against yourself. See if you can beat the version of you from last week, this way you always have your greatest competition: you. Many people will fail to find motivation; these people are only avoiding death for as long as they live. They are not actually living to their full potential.
Like Ayn Rand said "cheating life is not the same as avoiding death". She means the only true way to live is to have purpose, purpose that leads to accomplishments.
She would also state that this is the only way to ever find true joy in life. The world is built in the image of man, so determine what image you would like to have reflected in the world around you.
The actions and decisions you choose to make directly affect the direction and course that you follow in life. If you want to see a beautiful world, you have to look for beauty first in yourself.
If you want to see a world of possibility you have to see possibility within yourself. Everything you are starts as a single pulse of electricity through a single neuron in the brain.
